By accepting these terms and conditions, I hereby authorise the Head Coach to act for me according to their best judgment in any emergency requiring medical attention. I release Melbourne School of Tennis from any and all liability for injury or illness and costs, incurred while undertaking a class or lesson with Melbourne School of Tennis. I agree with all of Melbourne School of Tennis' policies as stated on the Policy page on this website. I acknowledge that I have read and accepted these.
